Who I am
Reset in Somerset is the private practice of Polly Haselton, a BABCP-accredited CBT therapist. I am the data controller for the information described here and am registered with the Information Commissioner’s Office. You can contact me at hello@resetinsomerset.co.uk.
What I collect, and why
- Enquiries. Your name, contact details and whatever you choose to tell me, so that I can reply and arrange a first session.
- Therapy records. If we work together, I keep contact details, session notes and correspondence, as my professional body and professional liability insurance requires me to do so.
- Payments and insurance. Details needed to invoice you or your insurer.
How it is kept
Client records are held in a secure patient management system. As a member of the BABCP I follow its code of ethics, which covers confidentiality and data protection.
Confidentiality and its limits
What you tell me is confidential. I would only share information without your agreement where I am required to by law or where there is a serious risk of harm to you or someone else, and I would talk to you first wherever possible.
Your rights
You can ask to see the information I hold about you, to have it corrected, and in some circumstances to have it deleted. If you are unhappy with how your information has been handled you can complain to the ICO at ico.org.uk.
This website
This site does not use advertising or tracking cookies. The map on the contact page is a drawing held on this site, so no map service is contacted when you view it.
Visits to this site are counted so that I can see which pages people find useful. No cookies are used and nothing is stored on your device. Each count records the page, the website or campaign link you came from, your country, and the type of device, browser and operating system. Your internet address is used for a moment to work out the country and to avoid counting you twice in a day; it is not kept, and nothing that is kept can identify you. The counting is done for me by Crux, who built this site.
Messages sent through the contact form are delivered to my email inbox by FormSubmit (formsubmit.co), a service that passes form messages on by email. If you would rather not use the form, you are welcome to email me directly.
